Lost Passport Reports:
Before reporting your lost or stolen passport, please ensure you have the Passport Number ready as this is required to make the report.
Obtaining a Motor Vehicle Collision Report
Motor Vehicle Collision Reports will only be released to individuals involved in the motor vehicle collision, a vehicle owner, or property owner.
If you require only the driver, vehicle and insurance information, this can be obtained free of charge. Individuals requiring a full copy or insurance companies and law firms must send in a written request, along with an original signed authorization and a fee of $50.00 (taxes included) payable to Belleville Police Service.
Officer note(s) and/or statement(s) can also be obtained at a cost of $20.00 for each statement requested. Accident Reconstruction reports may also be available.

Confirmation Letters
A Confirmation Letter verifies that a report has been filed with the Belleville Police Service. This will include basic information about an occurrence, based on the incident type and relevant details. If you require proof that an incident has been documented with the Belleville Police Service, a Confirmation Letter may be appropriate.
Individuals must attend the Belleville Police headquarters in person to request a report, for an occurrence which they are involved in.
